ALL VOTING MEMBERS WILL RECEIVE QUARTERLY REPORTS FROM THE BOARD OF DIRECTORS AND YEAR-END
FINANCIAL STATEMENTS.

Annual Membership Fee: $1,000.00

Associates will receive:

an invitation to one Five Element dinner at 1198 Ridge Road North with the students & faculty

attendance at our annual health fair or a continuing professional education day of your choice
compliments of Ongiara College

inclusion on the practitioners or company listing on website with link to your website(as appropriate)
